- Complete the Lifestyle Survey. Here is a link to the survey.
- Complete a 5-day Food Log. At the end of each day write down everything you ate and drank. Please include the approximate time. You can keep this in a email, word document or here is a link to my daily food log. Which ever is easiest for you.
- Schedule our meeting. I will be in contact with you shortly to schedule meeting time and date. I will also send you the above links in email format.
